Hospital Liaison Services
The Hospital Liaison staff person is responsible for evaluation of hospitalized individuals who are eligible or who are currently receiving CMH services. Staff participates in discharge planning while the individual is hospitalized, and also provides coordination of care to medical providers, referral to CMH programs or contract agencies upon discharge, and assistance with benefit applications. This service is provided at Port Huron Hospital , Caro Regional Hospital , Harbor Oaks Hospital , the Forensic Center , and Mount Pleasant Regional Center.
