Home Based Program

The Home-Based Program provides in-home services to children and adolescents age 4-18 years old and their families. The individuals in this program:

Have been diagnosed with a severe emotional disturbance
Are at imminent risk of out of home placement
Have tried less restrictive treatment interventions
Need a high level of intervention to avoid jeopardizing the family unit

CMH currently facilitates wraparound services within this program and also may include intervention within the community (school, work, juvenile justice system, etc.)
