Publications, Reports, and Notices

St. Clair County Community Mental Health (CMH) publishes a number of reports each year that may be of interest to the general public and to individuals who receive public services for mental health and/or substance use disorders. These publications are listed below. In addition, St. Clair County CMH is required to post a number of notices regarding the programs we offer.  These notices are listed at the bottom of this page.

The most recent annual report of St. Clair County CMH, published in May of 2013, is an overview of Fiscal Year 2012 (October 1, 2011-September 30, 2012).  The theme of the report, which includes data and feature articles, is "Spirituality: A Pathway to Recovery."

Contract Monitoring

St. Clair Contract Monitoring Annual Report
This report takes a look at how the contracted providers of the St. Clair CMH system performed with each other, as well as in each of the number of performance domains set out in state and federal regulations.

PIHP Contract Monitoring Annual Report
This report takes a look at how the three CMHs and the SUD providers performed in comparison with each other, as well as in each of the number of performance domains set out in state and federal regulations.



Quality Improvement

Quality Improvement Annual Summary Report 

This report details quality improvement initiatives in key areas of the CMH -- community relations, corporate compliance, finance, human resources and development, priveleging and credentialing, program development, regional impacts, recipient rights, systems improvements, utlization management and information systems.

Quality Improvement Plan

This report outlines both the quality of clinical care and the quality of administrative and non-clinical aspects of the organization, which support services to individuals served and their families in the community. Thumb Alliance Prepaid Inpatient Health Plan (PIHP)

Application for Renewal and Recommitment 2009-2014
The Thumb Alliance PIHP's Application for Renewal and Recommitment (ARR) to the Michigan Department of Community Health. The ARR outlines the goals of the Thumb Allinace PIHP, Lapeer, Sanilac, and St. Clair County CMHs, and the Coordinating Agency for Substance Use Disorders.

The ARR: Public Dialogue and Input

As part of the ARR process, the PIHP turned to the general public for review and input.

Customer Satisfaction Survey, Thumb Alliance 

This report explores how individuals who use CMH services throughout the Thumb Alliance, including St. Clair County CMH, feel about their experiences within the system.
